(1) serum cystatin C in mg/L
(2) serum creatinine in µmol/L
(3) history of renal transplant
(4) presence of spina bifida
(6) height in cm

= 75.94 / ((cystatin C)^1.17) * (factor 1 for renal transplant)
where:
• If renal transplant then factor 1 is 1.2, else 1.

= 507.76 * EXP(0.003 * (height)) / (((cystatin C)^0.635) * ((creatinine)^0.547)) * (factor 2 for renal transplant) * (factor for spina bifida)
where:
• If renal transplant then factor 2 is 1.165, else 1.
• If spina bifida present then factor is ((serum creatinine)^0.925)/40.45, else 1.
