Patient selection: age > 18 years
Parameters (using an inextensible steel measuring tape):
(1) age in years
(2) sex
(3) ethnicity
(4) calf circumference in cm (seated, maximum)
(5) upper arm circumference in cm (hanging loosely, midway between olecranon and acromion)
(6) thigh circumference in cm (standing, weight on opposite leg, both feet on floor, midway between iliac crest and knee)
(7) body mass index (BMI) in kg per square meter
(8) waist circumference in cm (just above the ilium, at the end of normal expiration)
