Wiley and Koepke reported an equation for estimating the volume of packed red blood cells needed to transfuse a premature neonate.
Parameters:
(1) hematocrit at baseline
(2) hematocrit target
(3) weight in kilograms
hematocrit rise =
= (target hematocrit) - (baseline hematocrit)
If the packed red cell volume of the blood product is 0.75:
mL of packed red blood cells transfused =
= 4/3 * (desired hematocrit rise) * (weight in kilograms)
If the packed red cell volume of the blood product is 0.66:
= 3/2 * (desired hematocrit rise) * (weight in kilograms)
where:
• The blood volume of a full-term neonate is about 85 mL / kg.
• For premature infants, the blood volume is 108 mL / kg.
