Patient selection: non-obese and obese adults, from 57 to 194 kg
Parameters:
(1) target area under the curve at 24 hours (75 mg•h/L)
(2) eGFR in mL per min, from 75 to 171 mL per min
eGFR used the MDRD equation de-indexed from 1.73 square meters BSA =
= (eGFR by MDRD in mL per min per 1.73 sq m) * (BSA in square meters) / 1.73
tobramycin dose in mg =
= (target area) * 6.33 * (1 + (0.0099 * ((eGFR) - 115)))
If the de-indexed eGFR is < 77 or > 171 then the dose estimate should be interpreted with caution.
