Description

The International League of Association for Rheumatology (ILAR) has listed criteria for the diagnosis of juvenile arthritis with enthesitis. The original criteria proposed in Santiago, Chile, in 1994 were modified at Durban, South Africa, in 1997.


 

Criteria for juvenile arthritis: onset before 16th birthday

 

Method 1 - all of the following:

(1) arthritis

(2) enthesitis

(3) none of the following:

(3a) presence of psoriasis in a first or second degree relative, confirmed by a dermatologist

(3b) presence of a systemic arthritis

 

Method 2 - all of the following:

(1) arthritis OR enthesitis

(2) :two or more of the following

(2a) sacroiliac joint tenderness AND/OR inflammatory spinal pain

(2b) presence of HLA-B27

(2c) family history involving one or more first or second degree relatives with an HLA-B27 related disease, confirmed by a physician

(2d) anterior uveitis (typically with pain, redness and/or photophobia)

(2e) onset of arthritis in a boy > 8 years of age

(3) none of the following:

(3a) presence of psoriasis in a first or second degree relative, confirmed by a dermatologist

(3b) presence of a systemic arthritis

 

where:

• Inflammatory spinal pain involves pain in the spine at rest, with morning spinal stiffness that improves with activity.

 

Additional descriptors:

(1) age of onset of arthritis and/or enthesitis

(2) pattern of arthritis at 6 months after disease onset (large vs small joint, limb predominance, specific joint)

(3) asymmetry of arthritis

(4) oligoarthritis vs polyarthritis

(5) presence of inflammatory bowel disease
