Description

Kakushima et al reported a score for distinguishing a duodenal adenoma from adenocarcinoma on endoscopy. The authors are from Shizuoka Cancer Center in Japan.

Patient selection: nonampullary duodenal epithelial tumor (NADET)

Parameter:

(1) lesion diameter in mm

(2) color

(3) macroscopic type

(4) nodularity

• Paris classification: Ip (protruded pedunculated); Is (protruded sessile); IIA (superficial elevated); IIc (depressed; mixed (more than one pattern).

total score =

= SUM(points for all of the parameters)

Interpretation:

• minimum score: 0

• maximum score: 5

• A score >= 3 is seen with a Vienna classification 4 (high-grade dysplasia) or higher.

• A score <= 2 is associated with a Vienna classification 3.

Performance:

• The sensitivity was 84% and specificity 0.90.
