Villa et al reported the ENDING-S (END-of-Life ScorING-System) for identifying a patient in the Intensive Care Unit (ICU) who is highly likely to die. Such a patient may be a candidate for palliative care. The authors are from the University of Florence, Careggi University Hospital, Harborview Medical Center and the University of Washington.
Patient selection: critically-ill patient, in ICU > 4 days
Parameters:
(1) length of stay in the ICU in days (LOS)
(2) number of days on mechanical ventilation (MV)
(3) number of days requiring vasoactive drugs (VD)
(4) sepsis
ENDING-S =
= (7.25 * (days on MV) / (LOS)) + (10.45 * (days on VD) / LOS)) + (0.3 * (LOS)) + (3 if septic)
Interpretation:
• A score > 11.5 was associated with death in the ICU.
Performance:
• The area under the ROC curve is 0.98. In a validation study the ROC was 0.79.
