Description

Kosuge et al identified electrocardiographic findings associated with takostubo cardiomyopathy (TC). These findings can help to exclude an acute anterior myocardial infarction. The authors are from Yokohama City University.

ECG features diagnostic for TC - both of the following:

(1) presence of ST-segment depression in aVR

(2) absence of ST-segment elevation in lead V1

Additional findings favoring TC over anterior AMI:

(1) absence of reciprocal changes

(2) longer maximal QTc (567 ms +/- 81), versus 489 +/- 61 in anterior AMI

Performance of the 2 diagnostic features:

• The sensitivity was 91% and specificity 96%

• The predictive accuracy is 95%.
