Shigellosis may be complicated by an encephalopathy that may range from mild to severe.
Features of Shigella-associated encephalopathy:
(1) altered level of consciousness
(2) lethargy and disorientation
(3) seizures
(4) coma
Ekiri syndrome (name given for disorder in Japanese children):
(1) rapid onset of neurologic abnormalities
(2) mild colitis
(3) significant cerebral edema
(4) high fatality rate
The diagnosis requires exclusion of alternative causes.
If the patient survives neurological residua may be present, such as hypothalamic hypopituitarism and impaired memory.
