Assaoui et al reported criteria for being able to wean a patient from extracorporeal membrane oxygenation (ECMO) placed for refractory cardiogenic shock. The authors are from University Pierre et Marie Curie and Hopital Europeen Georges Pompidou in Paris.
Patient selection: VA-ECMO placed for refractory cardiogenic shock
Weaning test: flow reduction to less than 1.5 L/min (minimal ECMO flow)
Parameters associated with successful weaning:
(1) aortic time-velocity integral (TVI) >= 10 cm
(2) left ventricular ejection fraction > 20-25%
(3) peak systolic velocity at lateral mitral annulus (TDSa or S') >= 6 cm/s
Performance:
• The area under the ROC curve is 0.93.
