Jenni et al reported echocardiographic criteria for the diagnosis of isolated ventricular non-compaction (IVNC). The authors are from University Hospital Zurich.
Criteria:
(1) absence of other cardiac abnormalities
(2) 2 layers in the left ventricle, with a thin compacted epicardial layer and a thick non-compacted endocardial layer (see ratio below)
(3) localization of findings to the mid-lateral, apical and mid-inferior areas
(4) evidence of deep perfused interrabecular recesses on color Doppler echocardiography
A ratio of (endocardial layer thickness) / (epicardial layer thickness) at end systole > 2 is diagnostic. A ratio < 2 is seen in conditions that are in the differential diagnosis.
Interestingly regional hypokinesia was not confined to the non-compacted segments.
