Hashimoto et al reported findings associated with lethal tachyarrhythmias in a patient with chronic kidney disease (CKD). These can help to identify a patient who may benefit from more aggressive management. The authors are from multiple institutions in Japan.
Patient selection: chronic kidney disease
Patient assessment: 24-hour Holter monitoring
Outcomes: tachyarrhythmias and/or sudden cardiac death
Predictors of lethal ventricular tachyarrhythmias:
(1) worst values of ambulatory-based late potentials
(2) nonsustained ventricular tachycardia (NSVT)
Worst late potentials:
(1) measurements when the RMS40 was the smallest over 24 hours
(2) considered positive with >= 2 out of 3 readings: fQRS > 135 ms, RMS40 < 15 µV, LAS40 > 30 ms
