Zhou et al reported ECG findings associated with acute total occlusion of the left main coronary artery without collateral circulation. The authors are from multiple institutions in China.
Patient selection: acute myocardial infarction
The electrocardiogram shows:
(1) ST segment acute myocardial infarction (STEMI)
(2) ST elevation in anterior/precordial (V2 to V4) through V6 (V3 and V4 in 100%)
(3) ST elevation in leads I and aVL (aVL 100%, I 92%)
(4) ST depression in inferior leads (II, III, aVF; all 100%)
Other findings:
(1) no ST deviation in aVR in 80%
(2) left anterior hemiblock in 68%
(3) right bundle branch block in 48%
The risk of sudden death is high.
