Description

Smith et al reported an equation for separating early repolarization from a subtle anterior ST-segment elevation myocardial infarction (STEMI). The authors are from Hennepin County Medical Center, University of Minnesota, Abbott Northwestern Hospital, Minneapolis Heart Institute, Mercy Medical Center and Biostatistical Consultanting.


Parameters:

(1) ST segment elevation in mm in lead V3 at 60 ms after the J-point

(2) QTc in ms

(3) R-wave amplitude in mm in lead V4

 

score =

= (1.196 * (ST segment elevation)) + (0.059 * (QTc)) - (0.326 * (R wave))

 

Interpretation:

• A score > 23.4 indicates STEMI.

• A score <= 23.4 indicates early repolarization.

 

Performance:

• The sensitivity was 86% and specificity 91%.
