Description

Peguero et al proposed a new ECG score for the diagnosis of left ventricular hypertrophy (LVH). The authors are from Columbia University and Mount Sinai Medical Center (Miami).

Patient selection: adult (mean 54 +/- 17 in test and 68 +/- 15 in validation groups)

Parameters:

(1) amplitude of the deepest S wave in any lead in mV

(2) amplitude of the S wave in lead V4 in mV

(3) sex

 

score in mV =

= (amplitude of deepest S wave) + (amplitude in V4)

Score for LVH

female

>= 2.3 mV

male

>= 2.8 mV

 

Performance:

• The area under the ROC curve reported as 0.85.

 

Macfarlane et al commented that different cut-offs may be necessary for patients less than 40 years of age. In addition, they commneted that race may also need to be considered.
