The Early Prediction of Decompensation (EPOD) score can be used to evaluate a patient with cirrhosis. This can help to identify a patient who is at risk for early decompensation. The authors are from multiple institutions in German, Spain and the United States.
Patient selection: cirrhosis
Parameters:
(1) serum albumin in g/dL
(2) platelet count per microliter
(3) serum total bilirubin in mg/dL
Parameter
Points
serum albumin
-0.55 * (albumin)
platelet count
-0.004 * (platelet count) /1000
serum total bilirubin
0.16 * (bilirubin)
EPOD =
= 4 * (SUM(points for the 4 parameters)+5.38)
Interpretation:
• The median score for low risk patients was around 9.
• The median score for high-risk patients was around 12.5.
Performance:
• The area under the ROC curve is 0.69.
Equation 7 can be used to estimate the probability of being compensated at a future time point.
