The early diastolic velocity of the mitral annulus (Ea) reflects left ventricular relaxation and can be used to estimate the pulmonary capillary wedge pressuer (PCWP). It is a preload independent index of left ventricular relaxation.
Analysis: Doppler ultrasonography
Parameters:
(1) early diastolic velocity of the mitral annulus (Ea) in cm/s
(2) early transmitral flow velocity in cm/s
ratio of E to Ea =
= E / Ea
PCWP in mm Hg =
= (1.24 * (E to Ea ratio)) + 1.9
Interpretation:
• An Ea <= 8 cm/s is associated with impaired left ventricular relaxation.
• An E to Ea ratio > 10 indicates a mean PCWP > 15 mm Hg.
