Diverticuli can occasionally occur in the duodenum. The diagnosis can be a challenge since many clinicians are unfamiliar with its various presentations.
Location: usually periampullary (second part of the duodenum)
Clinical features:
(1) variable abdominal pain
(2) variable acute pancreatitis
(3) variable bile duct obstruction with jaundice (Lemmel Syndrome)
(4) variable perforation with peritonitis
Laboratory findings:
(1) variable elevation of inflammatory markers (CRP, other)
(2) variable markers of obstructive jaundice or pancreatitis
The diagnosis is typically made on imaging studies that show a diverticulum arising from the duodenum.
