Description

McElvaney et al reported the Dublin-Boston prognostic score for patients with COVID-19. This is based on the change in the ratio of interleukin-6 to interleukin-10 over 4 days. The authors are from the Royal College of Surgeons in Ireland, Beaumont Hospital Dublin, and Brigham and Women's Hospital.


Patient selection: COVID-19

 

Parameters:

(1) serum interleukin-6 on day 0 (admission) in pg/mL

(2) serum interluekin-10 on day 0 in pg/mL

(3) serum interleukin-6 on day 4

(4) serum interleukin-10 on day 4

 

ratio of interleukin-6 to interleukin-10 on day 0 =

= (interleukin-6 on day 0) / (interleukin-10 on day 0)

 

ratio of interleukin-6 to interleukin-10 on day 4 =

= (interleukin-6 on day 4) / (interleukin-10 on day 4)

 

difference in ratio from day 0 to day 4 =

= (ratio on day 4) - (ratio on day 0)

 

Transformation of score:

(1) multiply by 2

(2) round to an integer

(3) MAX set to 2

(4) MIN set to -2

 

This results in a possible value of -2, -1, 0, 1 or 2.

 

Interpretation:

The higher the score the worse the prognosis.
