Description

Jensen et al evaluated peptic ulcers using a Doppler endoscopic probe (DEB). Detection of arterial blood flow using the probe is associated with high risk bleeding. The authors are from Ronald Reagan UCLA Medical Center, West Los Angeles Veterans Administration Medical Center and the University of California in Los Angeles.

Patient selection: bleeding peptic ulcer

High risk features for peptic ulcer bleeding:

(1) Forrest classification

(2) arterial blood flow by DEP at baseline

Forrest Classification

Risk

active arterial bleeding (IA)

high

nonbleeding visible vessel (IIA)

high

adherent clot (IIB)

moderate-to-high

oozing (IB)

moderate

flat spot (IIC)

moderate

clean base (III)

low

Forrest IA, IIA, and IIB were considered the major stigmata of recent hemorrhage (SRH).
