Documentation bias influences what gets documented in a given situation.


Bias in documentation may include:

(1) variation in when documentation is performed

(2) what gets documented

(3) how things get documented

(4) measurement error and other sources of variation in measurement

 

In some situations, this is a variation on target fixation, where focusing only on certain things and failing to see anything else.

 

Documentation bias can be reduced by using standards in how and what gets recorded.

 

Differential diagnosis:

(1) intentional fraud (recording incorrect data or preforming incorrect calculation for profit)

(2) white washing
