Description

A scoring system can be used to determine if an outpatient is a candidate for fast track discharge after having had ambulatory surgery.

7 parameters:

(1) level of consciousness

(2) physical activity

(3) hemodynamic stability

(4) respiratory stability

(5) oxygen saturation status

(6) postoperative pain assessment

(7) postoperative emetic symptoms

Parameter

Finding

Points

level of consciousness

responsive only to tactile stimulation

0

arousable with minimal stimulation

1

awake and oriented

2

physical activity

unable to voluntarily move extremities

0

some weakness in movement of extremities

1

able to move all extremities on command

2

hemodynamic stability

blood pressure +/- 50% of baseline

0

blood pressure +/- 30% of baseline

1

blood pressure +/- 15% of baseline

2

respiratory stability

dyspneic with weak cough

0

tachypnea with good cough

1

respiratory rate 10-20 breaths per minute, and able to breath deeply

2

oxygen saturation status

< 90% with supplemental oxygen

0

requires supplemental oxygen to maintain saturation > 90%

1

maintains > 90% while breathing room air

2

postoperative pain assessment

persistent moderate-to-severe pain

0

moderate-to-severe pain controlled with intravenous analgesics

1

no or mild discomfort

2

postoperative emetic symptoms

persistent moderate-to-severe nausea and vomiting

0

transient vomiting or retching controlled with intravenous antiemetics

1

no or mild nausea with no active vomiting

2

score =

= SUM(points for the 7 parameters)

Interpretation:

• minimum score: 0

• maximum score: 14

• The patient is a candidate for fast track discharge if the score >= 12 and no parameter has 0 points.
