Diphallia (penile duplication) is a very rare congenital malformation.
Classification of Gyftopoulos et al:
(1) true diphallia (2 true anatomic penises)
(1a) both approximately equal in size
(1b) one normal size and one rudimentary
(2) bifid
(2a) complete/proximal (forks from a common base)
(2b) partial/distal (distal fork at the level of the glans)
Pseudodiphallia refers to accessory penile tissue that does not show the normal anatomic appearance of a penis.
The presence of diphallia often occurs together with genitourinary or anorectal malformations including:
(1) bladder duplication
(2) bifid scrotum
(3) imperforate anus
(4) lumbosacral abnormalities
