Description

A patient with chronic renal failure may have muscle weakness and cramping for several reasons.


 

Differential diagnosis of muscle weakness in a patient with chronic renal failure:

(1) uremic myopathy

(2) myopathy associated with dialysis

(3) drug-induced

(4) anemia

(5) uremic polyneuropathy

(6) secondary hyperparathyroidism

(7) other electrolyte disorders (potassium, magnesium, phosphate)

(8) malnutrition

(9) ischemic myopathy

(10) depression

 

Features of uremic myopathy:

(1) proximal muscle weakness

(2) preserved deep tendon reflexes

(3) muscle biopsy shows selective Type II fiber atrophy
