Serum transferrin may be reduced for a number of reasons.
Differential diagnosis of hypotransferrinemia:
(1) nephrotic syndrome
(2) protein-losing enteropathy
(3) protein malnutrition
(4) certain hematologic malignancies (erythroleukemia, multiple myeloma)
(5) chronic liver disease
(6) autoantibody to transferrin
(7) hereditary atransferrinemia (heterozygotes show reduced transferrin, homozygotes show absent transferrin)
