Description

Yu et al developed a scoring test to identify patients with DIC based on the clinical presentation and commonly used laboratory tests. The authors are from the University of Massachusetts Memorial Health Care in Worchester.

Clinical setting:

(1) obstetric accidents (amniotic fluid embolism, placental abruption, retained fetus syndrome, eclampsia, abortion)

(2) intravascular hemolysis (hemolytic transfusion reaction, minor hemolysis, massive transfusions)

(3) septicemia

(4) viremias

(5) metastatic malignancy

(6) leukemia (acute promyelocytic, acute myelomonocytic)

(7) burns

(8) crush injuries and tissue necrosis

(9) trauma

(10) acute liver disease

(11) prosthetic devices (Leveen shunt, Denver shunt, aortic balloon assist device)

(12) vascular disorders

(13) disorders associated with low-grade DIC (cardiovascular disease, autoimmune disease, renal vascular disorders, hematologic disorders, inflammatory disorders)

 

Thrombohemorrhagic events: one or more of the following:

(1) petechiae

(2) purpura

(3) hemorrhagic bullae

(4) acral cyanosis

(5) frank gangrene

(6) bleeding or oozing from wounds, incision sites, venipuncture sites, etc

(7) large subcutaneous hematomas

(8) deep tissue bleeding

(9) thrombosis

 

DIC score =

= SUM(points for all 8 parameters)

 

Interpretation:

• minimum score: 0

• maximum score: 8

• A score >= 5 is needed for the diagnosis of DIC.
