Dudley et al developed a simple scoring system for diagnosing primary (essential) thrombocythemia.
Patient selection: Patients had a platelet count > 600,000 per µL
Finding
Points
splenic enlargement on scan
2
unstimulated BFU-E derived colonies present
elevated platelet ATP/ADP ratio (> 4 standard deviations above mean)
1
elevated platelet distribution width (> 2 standard deviations above mean)
clinical ischemia present
where:
• BFU-E indicates erythroid colony forming units from peripheral blood
• The ATP/ADP ratio indicates the ratio of platelet ATP to ADP (platelet nucleotide ratio).
score =
= SUM(points for 5 findings)
Interpretation:
• A score >= 3 indicates primary thrombocythemia.
• A score < 3 indicates secondary or reactive thrombocytosis.
