Valent et al reported criteria for the diagnosis of basophilic leukemia. The authors are from multiple institutions in Europe and the United States.
Criteria for basophilic leukemia:
(1) hyperbasophilia (absolute basophil count >= 1,000 per µL) with clonal myeloid disease
(2) percent basophils in the differential count >= 40%
(3) presence of myeloblasts and metachromatic blasts (>= 20% in acute, < 20% in chronic)
Secondary:
(1) CML (with Ph-chromosome or BCR-ABL1)
(2) myeloproliferative neoplasm (MPN-baso)
(3) myelodysplasia (MDS-baso)
(4) acute myeloid leukemia (AML-baso)
(5) T-cell neoplasm
Basophilic leukemia is diagnosed when other causes of hyperbasophilia have been excluded.
