Isolated otolith dysfunction can be diagnosed if certain criteria are present. The authors are from Seoul National University Hospital in South Korea.
The patient may present with dizziness with or without drop attacks.
Symptoms:
(1) non-spinning vertigo
(2) horizontal floating
(3) flipping over (upside down)
(4) lateral translation
(5) up-down vibration
(6) tilt
Testing - all of the following:
(1) normal caloric test
(2) normal video head impulse test
(3) one or more abnormal otolith tests:
(3a) abnormal ocular vestibular evoked myogenic potential
(3b) abnormal cervical vestibular evoked myogenic potential
(3c) abnormal subjective visual vertical
Criteria for definite isolated otolith dysfunction:
(1) symptomatic)
(2) positive by testing
Criteria for probable isolated otolith dysfunction by symptoms:
(1) symptomatic
Criteria for probable isolated otolith dysfunction by testing:
(1) positive by testing
