Description

Asano et al reported diagnostic criteria for allergic bronchopulmonary mycosis (ABPM) for a patient without cystic fibrosis.


Diagnostic criteria:

(1) previous or current history of asthma or asthma-like symptoms

(2) eosinophilia (absolute eosinophil count >= 500 per µL) in the peripheral blood

(3) elevated serum IgE concentration (>= 417 IU/mL)

(4) immediate cutaneous hypersensitivity to filamentous fungi OR specific IgE for filamentous fungi

(5) presence of precipitins for filamentous fungi

(6) filamentous fungus seen in bronchial alveolar fluid OR cultured from sputum

(7) presence of fungal hyphae in a bronchial mucus plug

(8) central bronchiectasis on CT

(9) presence of mucus plugs in central bronchi (expectorated, on bronchoscopy, in CT images)

(10) high attenuation mucus in the bronchi on CT

The filamentous fungi seen in various criteria should be identical.

Interpretation:

minimum score: 0

maximum score: 10

Allergic bronchopulmonary mycosis can be diagnosed if >= 6 of the criteria are present.
