The derived neutrophil to lymphocyte ratio is based on the fact that neutrophils and lymphocytes are the predominant components of the differential count.
Parameters:
(1) percent neutrophils in the peripheral blood
presumed lymphocyte percentage =
= 100 - (percent neutrophils)
dNLR =
= (percent neutrophils) / (100 - (percent neutrophils)
The performance of this ratio depends on neutrophils and lymphocytes being the dominant forms with all other cells in small numbers.
Interpretation:
• An elevated dNLR is associated with a worse outcome.
• For diffuse large B-cell lymphoma an elevated dNLR is one >= 1.8.
The minimum percent neutrophils to exceed the cutoff X can be calculated from:
percent neutrophils = 100 * (1 / (1 + X))
