Description

A patient who has received a crotaline snakebite may develop a delayed coagulopathy even after initial antivenom therapy.


Crotaline snakes: rattlesnake, cottonmouth, copperhead

 

Features of the coagulopathy:

(1) clinical bleeding

(2) thrombocytopenia

(3) hypofibrinogenemia

(4) elevated fibrin split products

 

The onset may be 2 to 7 days after the bite.

 

It may occur in a patient who has received antivenom. The antivenom (CroFab) uses the Fab fragment of the immunoglobulin and has a short half-life. A patient with delayed onset usually receives additional antivenom.

 

The coagulopathy can be recurrent.

 

A patient should take bleeding precautions for 2 weeks after the snakebite with no contact sports, elective surgery or dental work.
