An elevator raises people or materials to different levels in a building by moving up and down a shaft on a platform or compartment.
Risk factors for a fatal elevator accident:
(1) risky behavior (elevator surfing, other)
(2) worker engaged in elevator construction or maintenance
(3) defect in design or manufacture (failure to have a full-length door, etc)
(4) absence/failure of safety measure
(5) elevator malfunction
(6) intoxication or inattention
Cause of death may include:
(1) asphyxia
(2) blunt trauma
(3) avulsion or caught between injuries
(4) drowning
(5) fire
