Dalteparin sodium (Fragmin, Pharmacia & Upjohn) is a low molecular weight heparin (LMWH) with activity measured in WHO international units of anti-Factor Xa activity.
2,500 IU anti-Factor Xa activity = 16 mg dalteparin
1 mg dalteparin = 156.25 IU anti-Factor Xa activity
Dalteparin is supplied in prefilled syringes or in multi-use vials containing 95,000 IU.
The prefilled syringes contain either:
(1) 2,500 IU per syringe
(2) 5,000 IU per syringe
(3) 7,500 IU per syringe
The vials either contain:
(1) 9.5 mL with 10,000 IU/mL, or
(2) 3.8 mL with 25,000 IU/mL
