Description

Aviram et al identified changes on CT pulmonary angiography that are associated with mortality for a patient with pulmonary embolism. These can help to identify a patient who may benefit from more aggressive management. The authors are from Tel Aviv University.

Patient selection: acute pulmonary embolism

Outcome: 30-day mortality

Exam: CT pulmonary angiography

Parameters:

(1) left atrial volume in mL

(2) left venticular volume in mL

(3) ratio of right atrial to left atrial volumes

total number of risk factors =

= SUM(points for all 3 parameters)

Interpretation:

• minimum number of risk factors: 0

• maximum number of risk factors: 3

• The risk of mortality increases with the number of risk factors.

• Left atrial volume was the best predictor with an adjusted hazard ratio of 2.4.
