Description

Wu et al used computed tomography (CT) to identify patients with gangrenous cholecystitis. Gangrenous cholecystitis is associated with high morbidity and mortality if not detected early. The authors are from Chang Gung University in Taiwan.


Patient selection: acute cholecystitis

 

Findings associated with gangrenous cholecystitis:

(1) perfusion defect (sensitivity 71%, specificity 100%)

(2) pericholecystic stranding

(3) absence of gallstones

 

The combination of perfusion defect or no gallstones was reported as having a sensitivity of 88% and specificity of 100% for gangrenous cholecystitis.

 

Additional findings with low sensitivity but high specificity:

(1) mucosal hemorrhage

(2) mucosal sloughing

(3) wall irregularity

(4) pericholecystic abscess

(5) gas formation

(6) portal venous thrombosis
