Description

Elmariah et al reported the CRRAC score for evaluating a patient undergoing percutaneous balloon aortic valvuloplasty. This can help to identify a patient who may require more aggressive management. The authors are from Mount Sinai School of Medicine, Massachusetts General Hospital, Brigham and Women's Hospital and Harvard Medical School.

Acronym: CRRAC

Patient selection: percutaneous balloon aortic valvuloplasty

Outcome: 30-day mortality

Parameters:

(1) critical status (C)

(2) renal dysfunction (R) based on serum creatinine in mg/dL

(3) pre-procedural right atrial pressure in mm Hg (RA)

(4) cardiac output (C)

Critical status - one or more of the following:

(1) ventricular tachycardia or fibrillation

(2) aborted sudden death

(3) preoperative ventilation

(4) preoperative inotropic support

(5) intra-aortic couterpulsation

(6) preoperative anuria or oliguria

Interpretation:

minimum score: 0

maximum score: > 50 (depending on right atrial pressure)

A score >= 20 is associated with an increased risk for mortality.
