Description

Zen et al reported criteria for Minimal Disease Activity (MDA) in a patient with Systemic Lupus Erythematosus (SLE). The authors are from the University of Padova in Italy.

Patient selection: systemic lupus erythematosus (SLE)

SLEDAI-2K Score During Annual Visits

Number of Annual Visits

Designation

0

in 3 annual visits

clinical quiescent (CQD)

1

in 1 or more annual visits

minimal disease activity (MDA)

>= 2

in >= 2 annual visits

chronic active (CAD)

>= 2

in 1 out of 3 annual visits

relapsing-remitting (RRD)

Alternatively:

SLEDAI-2K Score in 3 Annual Visits

Designation

0 in all 3 visits

clinical quiescent (CQD)

1 in >= 1 with no >= 2 visits

minimal disease activity (MDA)

>= 2 in 1 visit

chronic active (CAD)

>= 2 in >= 2 visits

relapsing-remitting (RRD)

A flare was defined as an increase in SLEDAI-2K >= 4 since the previous visit.

Criteria for minimal disease activity - all of the following:

(1) SLEDAI-2K score 1

(2) excluding serology (anti-nuclear antibodies and complement)

(3) with or without antimalarial medication

(4) with or without immunosuppressive drugs (not exceeding maximum recommended dose)

(5) with or without low dose prednisone (<= 5 mg per day)

The SLEDAI-2K of 1 point could be seen with one of the following:

(1) fever (1 point)

(2) thrombocytopenia (platelet count < 100,000 per µL; 1 point)

(3) leukopenia (WBC count < 3,000 per µL; 1 point)
