Description

Yamamoto and Oiwa reported 7-items that be used to make the diagnosis of polyarteritis nodosa (PAN). The authors are from Hiroshima Citizens Hospital and Kawasaki Medical School in Japan.


Patient selection: vasculitis

 

Items:

(1) mononeuropathy multiplex

(2) gastrointestinal vasculitis (intestinal ischemia, vasculitis of gallbladder or appendix)

(3) negative myeloperoxidase (MPO) ANCA

(4) urine protein less than 2+

(5) (unexplained fever >= 38°C for >= 7 days) OR (weight loss >- 4 kg since disease onset not explained by other causes)

(6) aneurysms, stenosis or occlusion of visceral arteries not due to other causes

(7) granulocytes or mixed leukocyte infiltrates in the wall of small or medium arteries

 

The presence of >= 4 items had a sensitivity of 92% and specificity of 92% for PAN.
