Description

Tuncer et al listed criteria for describing the clinical outcome for a patient following therapy for thrombotic thrombocytopenic purpura-hemolytic uremic syndrome (TTP-HUS). The authors are from the University of Alabama at Birmingham.


 

Therapy consisted of therapeutic plasma exchange.

 

Parameters:

(1) platelet count

(2) serum LDH

(3) death

(4) duration of response

 

Criteria for response #1 - both of the following for >= 2 days after completing plasma exchange:

(1) platelet count >= 150,000 per µL (lower limit of normal reference range)

(2) serum LDH < 350 IU/L (upper limit of the normal reference range)

 

Criteria for response #2:

(1) clinically stable

(2) discharged with a platelet count < 150,000 per µL

(3) followup platelet count >= 150,000 per µL

(4) I am assuming that the LDH is normal

 

Criteria for nonresponse:

(1) failure to achieve a response (as defined above)

(2) death

 

Criteria for remission: persistence of response during the followup period, with no need for additional therapy

 

Criteria for relapse: new episode of TTP-HUS >= 4 weeks after obtaining an initial response
