Shaikh et al defined criteria for bacteriuria, pyruia and a urinary tract infection (UTI) in a child. The authors are from the University of Pittsburgh, Ohio State University, Duquesne University and the University of Wisconsin.
A urinary tract infection may affect the kidney or urinary bladder or both.
Criteria for a urinary tract infection - all of the following:
(1) bacteriuria
(2) pyuria
(3) symptoms (fever, dysuria, frequency, etc)
Specimen
Culture Criteria for Bacteriuria
suprapubic aspirate
any
catheterized specimen
>= 50,000 colony forming units (CFUs) per mL
clean catch urine
>= 100,000 colony forming units per mL
A culture from a perineal bag is more likely to yield a contaminant.
Specimen
Counting Method
Criteria for Pyuria
unspun urine
counting chamber
>= 10 per µL
spun urine
chamber or smear
>= 5 per high power field
where:
• CFU are counted per mL while WBC are counted per µL in an unspun sample.
