Schroeck et al reported criteria for appropriate antibiotic therapy in a patient with sinusitis. The authors are from VA Western New York Healthcare System, University of Buffalo, Erie County Medical Center and East Orange VA.
Criteria for the diagnosis of sinusitis:
(1) high fever (> 39°C, > 102°F)
(2) purulent nasal discharge OR facial pain compatible with sinusitis
(3) lasting >= 3 consecutive days
Antibiotic therapy is appropriate if the patient meets the clinical criteria for sinusitis.
Imaging studies of the sinuses would support the clinical diagnosis.
Appropriate antibiotic therapy:
(1) amoxicillin
(2) doxycycline or respiratory fluoroquinolone (levofloxacin, moxifloxacin) if penicillin-allergic
