Roach et al listed criteria for biochemical failure following therapy in a patient with prostate cancer. This is based on the criteria from the American Society for Therapeutic Radiation and Oncology (ASTRO) consensus conference.
Criteria:
(1) 2 consecutive and significant PSA increases
(2) each separated by at least 1 month
Previous PSA
Significant Increase
<= 1.5 ng/mL
>= 0.3 ng/mL
> 1.5 ng/mL
>= 0.20 * (previous PSA)
increase after first interval =
= (PSA in ng/mL at end of first interval) - (PSA at baseline)
increase after second interval =
= (PSA in ng/mL at end of second interval) - (PSA at end of first interval)
Exception: A patient with a slowly rising PSA (< 20% increase per year) was classified as not having biochemical failure as long as the PSA was < 4.0 ng/mL.
Limitations:
• This may not be applicable in a patient receiving hormonal therapy.
