Description

Rangel et al listed criteria for identifying a trauma patient who should be evaluated for ventilator-associated pneumonia (VAP). The authors are from the University of Cincinnati.


 

Patient selection: trauma patient receiving ventilator support

 

Criteria - both of the following:

(1) 2 or more findings from Group A

(2) 2 or more findings from Group B

 

Group A:

(1) pulmonary infiltrate on chest imaging study

(2) PaO2 to FIO2 ratio < 300 mm Hg

(3) purulent secretions from endotracheal aspirate

 

Group B:

(1) core body temperature <= 36°C or >= 38.3°C

(2) WBC < 5,000 per µL or > 10,000 per µL or >= 10% bands in the peripheral blood differential

(3) heart rate > 90 beats per minute

(4) respiratory rate > 20 breaths per minute OR PaCO2 < 32 mm Hg (hyperventilation)

(5) unexplained mean arterial pressure (MAP) < 70 mm Hg

(6) unexplained change in mental status

(7) unexplained ileus
