Description

Potts et al listed criteria for the diagnosis of severe alcoholic hepatitis. The authors are from Brighton and Sussex University in Brighton and King's College Hospital in London.


Criteria for severe alcoholic hepatitis - all of the following:

(1) jaundice (serum total bilirubin > 80 µmol/L)

(2) INR >= 1.5 (coagulopathy)

(3) modified discriminant function of Maddrey (Carithers et al) >= 32

(4) long-standing alcohol intake (male > 70 g/day, female > 50 g/day) "until at least" 6 weeks prior to admission (any abstinence < 6 weeks)

(5) one or more of the following:

(5a) ratio AST to ALT >2 on admission

(5b) enlarged, tender liver with peripheral leukocytosis

(5c) hepatic encephalopathy on admission

(5d) severe alcohol hepatitis on liver biopsy
