Ohara et al listed diagnostic criteria for IgG4-related sclerosing cholangitis.

The authors are from the multiple associations in Japan (Research Committee of IgG4-related Diseases; Research Committee of Intractable Diseases of Liver and Biliary Tract; Ministry of Health, Labor and Welfare; Japan Biliary Association).


Patient selection: sclerosing cholangitis

 

Diagnostic criteria for IgG4-related sclerosing cholangitis:

(1) characteristic findings on biliary imaging studies

(2) elevated serum IgG4 concentration

(3) co-existence of IgG4-related disease elsewhere (pancreas, etc)

(4) characteristic histopathologic features (lymphoplasmacytic infiltrate with IgG4 positive plasma cells, fibrosis, etc)

(5) clinical response to corticosteroid therapy (optional)

 

An additional criteria might be exclusion of alternative explanations for sclerosing cholangitis.
