Description

McGeer et al reported criteria for the diagnosis of infections within long-term care facilities. One category is systemic infection. The authors are from Mount Sinai Hospital (Toronto), Centers for Disease Control, Yale-New Haven Hospital, University of California at San Diego, University of Manitoba, Ontario Nursing Home Association, Ontario Ministry of Health, Clarkson Hospital (Omaha) and the Hospital for Sick Children in Toronto.


Patient selection: long-term care facility

 

Categories:

(1) primary bloodstream infection

(2) unexplained febrile episode

 

Criteria for primary bloodstream infection - one of the following:

(1) >= 2 blood cultures positive for the same organism

(2) all of the following:

(2a) 1 blood culture positive for an organism not thought to be a contaminant

(2b) symptomatic (drop in systolic blood pressure > 30 mm from baseline; body temperature >= 38°C; new hypothermia (< 34.5°C); worsening mental or functional status.

 

If the patient has a known site of infection (pneumonia, urinary tract, etc) then a bloodstream infection is considered secondary and the infection is designed based on the known site.

 

Criteria for unexplained febrile episode - both of the following:

(1) documented body temperature >= 38°C >= 2 times >= 12 hours apart over a 3 day period

(2) exclusion of infectious and noninfectious causes (no explanation)
