Lereuz-Ville et al listed criteria for symptomatic infection of a fetus with cytomegalovirus (CMV). Patient selection: neonate or abortus from mother with evidence of CMV during pregnancy
Clinical findings indicative of symptomatic disease:
(1) intrauterine growth retardation (IUGR)
(2) petechiae and/or purpura
(3) microcephaly
(4) seizures
(5) lethargy and/or hypotonia
(6) poor suck
(7) hepatosplenomegaly
(8) hearing loss
Imaging findings indicative of symptomatic disease:
(1) multiple intracranial calcifications
(2) periventricular hyperechogenicity
(3) severe ventriculomegaly (> 15 mm)
Laboratory findings indicative of symptomatic disease:
(1) thrombocytopenia
Postmortem neuropathologic findings indicative of symptomatic disease:
(1) microcephaly
(2) ventriculomegaly
(3) white matter necrosis
(4) vasculitis
(5) encephalitis
A neonate was considered asymptomatic if all of the following are normal:
(1) clinical exam
(2) normal platelet count
(3) normal hearing
(4) normal imaging
Findings NOT considered indicative of symptomatic disease:
(1) pseudocysts in the germinal matrix
(2) echogenic lenticulostriated vessels
(3) mild ventriculomegaly
