Description

La Grenade et al proposed criteria for diagnosis of infective dermatitis (ID) associated with HTLV-I infections. The authors are from the University of West Indies and the National Cancer Institute.


 

Major criteria:

(1) presence of eczema in >= 2 body sites (see list below)

(2) one or both of the following:

(2a) chronic watery nasal discharge without other signs of rhinitis

(2b) crusting of the anterior nares

(3) both of the following

(3a) chronic relapsing dermatitis with prompt response to appropriate therapy

(3b) prompt relapse/recurrence on withdrawal of therapy

(4) onset in childhood

(5) HTLV-I antibody seropositivity

 

Body sites evaluated for eczema:

(1) scalp

(2) axilla and groin

(3) external ear and retroauricular areas

(4) eyelid margins

(5) paranasal skin (see criteria 2b, above)

(6) neck

 

Criteria for diagnosis of infective dermatitis:

(1) presence of major criteria 1, 2 and 5

(2) presence of either major criteria 3 or 4

 

Minor criteria for diagnosis:

(1) positive cultures for Staphylococcus aureus or Streptococcus pyogenes (beta-hemolytic Group A) from the skin or anterior nares

(2) generalized fine papular rash in more severe cases

(3) generalized lymphadenopathy with dermatopathic lymphadenitis

(4) anemia

(5) elevated ESR

(6) hyperimminoglobulinemia (with IgD and IgE)

(7) elevated CD4 lymphocyte count, CD8 lymphocyte count, and CD4-to-CD8 ratio
