Description

Kuiper et al reported criteria for biochemical response to ursodeoxycholic acid (UDCA) therapy in a patient with primary biliary cirrhosis (PBC). This can help to identify a patient who may require more aggressive management. The authors are from the Dutch PBC Study Group.

Patient selection: primary biliary cirrhosis following therapy with ursodeoxycholic acid (UDCA) for a year or more

Parameters:

(1) serum total bilirubin in mg/dL (abnormal if above the upper limit of the normal reference range)

(2) serum albumin in g/dL (abnormal if below the lower limit of the normal reference range)

Early PBC is associated with both parameters being normal at baseline and after therapy. This group had a survival comparable to the general population.

A patient who responds to therapy tends to have a better prognosis than someone who does not.
